Scars occur when regions of your skin are replaced by a fibrous tissue after you experience an injury. The process of scar formation is known as fibrosis.
It is a natural step in the tissue repair and wound healing process. All external wounds heal with some degree of scarring.
Manuka honey, made by honeybees that have fed on the nectar of New Zealand’s native, wild manuka flower, contains an abundance of health benefits. The bees transfer a compound known as methylglyoxal from manuka nectar into the honey. Methylglyoxal is rich in antimicrobial properties.
Does Manuka Honey Reduce the Unattractiveness of Scarring?
Manuka honey is known and proven to an effective treatment method for acute and chronic wounds thanks to its antibacterial and anti-inflammatory properties. On the strength of this fact, a group of researchers performed a study in 2016 to determine if manuka honey can decrease scar formation and improve scars appearance.
21 patients completed the experiment. They all received an 8 cm incision. There were two groups. The first group was instructed to apply manuka honey paste on the incision site daily for 4 weeks. The second group was the control group who would not be applying honey to their wound.
The scarring was then analyzed by at 4 weeks and 8 weeks after the surgery by a blinded observer and by the patients themselves.
They did not observe any significant difference in scar appearance between those who applied honey and the control group.
The findings surprised the researchers since they were aware of the animal and histological studies which have previously demonstrated that application of honey improves epithelial repair. They concluded that the cause of the discordance between their findings and previous animal and histological studies was unclear and required further evaluation.
Honey and the Role of Acidity on the Wound Environment
A study sought to investigate the effect 4 different wound dressings would have on the pH levels of a wound. Manuka honey was one of the wound dressings examined by the study.
Changes in pH were measured in real time and the effect of local buffering observed.
The study found that all the examined wound dressings had a low pH (below pH 4). It concluded that the low pH (strong acidic nature) of these wound dressings indicated that they played a role in wound healing.
A review compiled and summarized what medical/scientific have to say about the role surface and pH environment play in the wound healing process. The review found that studies have proven that an acidic environment assists wound healing by controlling wound infection, releasing oxygen, altering protease activity, increasing antimicrobial activity, enhancing epithelization and angiogenesis, and reducing the toxicity of bacterial end products.

Honey and Epidural Fibrosis
Epidural fibrosis occurs when there is excessive production of scar tissue near a nerve’s root. It occurs most commonly after spinal surgery. It may occur in both successful and failed back surgeries. Complicated back surgeries are more likely to result in epidural fibrosis than less complicated ones.
Doctors can diagnose epidural fibrosis using either an MRI or CT scan. Symptoms of the problem include moderate to severe pain in your back and a noticeable pain increase weeks or months after surgery.

The difference between the two groups was statistically significant.
The study concluded that manuka honey does indeed reduce epidural fibrosis in rats post-laminectomy. The researchers interpreted their findings as proof that manuka honey may be safely used in clinics to apply on surgical wounds, that it does play a role in preventing development of epidural fibrosis after a laminectomy.
